Job Description
Responsible for completing assessment for admissions and readmissions, initial and ongoing assessment of residents with changing conditions, assess learning needs of residents and families and carries out teaching as indicated, maintain ongoing communication with primary care providers (PCP), provide timely and ongoing information for MDS assessments, perform urinary catheter changes, and more. Experience required 3 years of long-term care management experience required 2 years of acute care experience required
